The cost will vary depending on what is involved.  Mediation is charged at an hourly, half day or full day rate.

It depends on the process required and the availability of the parties.   Pre-mediation meetings are held with each of the parties.  Then a mediation date is arranged.   More than one mediation meeting may be required.

Not necessarily, although meeting in person can speed up the process of reaching a lasting agreement.
